NPMD is a secure document service operated by All Seniors Foundation, a non-profit organization. SMS messages on this program are sent under the All Seniors Foundation A2P 10DLC brand (the “NPMD Secure Document SMS” campaign).
This page documents the exact opt-in disclosure used for transactional SMS messages that notify recipients about secure documents requiring review or signature. The program is not used for marketing or promotional messages.
Where the opt-in appears
- A clinic user prepares a secure document or consent packet for a patient, attorney, or authorized recipient.
- Before SMS delivery is selected, the recipient's mobile number is confirmed and the disclosure below is presented as the SMS consent language for secure document notifications.
- The recipient may choose email instead. SMS consent is optional and is not required to receive care, services, or documents.
- If SMS is selected, NPMD sends only transactional signing notifications and reminders with STOP and HELP instructions.

The SMS checkbox is unchecked by default; a recipient or authorized clinic user must actively select SMS after confirming the mobile number, and email remains available as an alternate delivery method.
