NPMD Patient Care SMS
Appointment and follow-up opt-in proof
This page documents the staff-assisted chart workflow for appointment and follow-up choices. It is not a public self-enrollment form or proof that these purposes are already covered by a carrier-approved filing.
Consent-recording workflow
- Staff confirms the patient's current SMS-capable mobile number.
- Staff presents or reads the full disclosure shown below.
- The patient or authorized representative may choose appointment texts, follow-up texts, signature-request texts, any combination, or none. Another contact method remains available.
- Staff identifies who consented, confirms the disclosure attestation, and enables only each selected off-by-default category in the chart.
- NPMD binds each category to the current number and records the staff user, timestamp, disclosure version, and attested party. A phone-number change turns texting off until renewed consent is recorded.
Illustrative chart consent state
Staff confirms the current mobile number in the authenticated chart. This public example contains no patient data or working consent controls.
NPMD may send the selected transactional text-message categories, including appointment confirmations and reminders, follow-up scheduling reminders, and secure document review or signature requests. Message frequency varies by care event or document packet. Message and data rates may apply. Reply STOP to opt out or HELP for help. Consent is not required to receive care or services. Staff selects Patient or Authorized representative, then confirms: I confirmed the current mobile number, presented the NPMD SMS disclosure, and the patient or authorized recipient affirmatively agreed to the category I am enabling.
